Join a growing, innovative and caring organization that provides services to older adults and persons with disabilities.   Family Lifeline, a non-profit organization, is looking for a Home Care Services Coordinator to work collaboratively with the Home Care Team to ensure individuals receive high quality personal care services.  Essential job duties:  Coordinate and schedule care providers and trouble shoot concerns.  Assist with recruitment and on-boarding of care providers.  Provide marketing/outreach support.  Provide data entry and chart documentation support. Establish and maintain positive relationships with care recipients, their families and Family Lifeline staff.  Demonstrate flexibility and cooperation. Rotate on-call responsibilities. Minimum 1 year experience working in home care with Medicaid and licensing, preferred. A self-motivated, detail driven, problem solver with excellent administrative, customer service, and communications skills.  Proficient in Microsoft Office and ability to learn new database systems. 2 year degree preferred.  Current Virginia driver’s license; and reliable transportation.  Send resume and cover letter to HR Dept.
